Private Consultations

A private consultation can be booked via our contact page. Consultation is available both in clinic each Monday or via the telephone at a time which mutually suites.

Prior to your consultation, it is required that you fill out a comprehensive Pre-Consultation Questionaire. The questionaire can be ordered via the button below - there is no charge for the questionaire. You will be sent both the questionaire and a New Client Booklet which you must read before your first consultation. Just click on the 'Add To Cart' button below to access your free questionaire. When you have filled it out completely, save it to your desktop then attach it to an email and send it back to:
